2023 LibQUAL+ Online Survey

1. Notes to Potential Respondents:
   1. This online survey is only open to EdUHK students and staff. Other library users including Alumni, School Teachers, Staff Dependants, other Free or Paid External Members, etc. please DO NOT proceed.
   2. This is an anonymous survey. No personal information will be collected.
   3. If you are interested in joining the Lucky Draw, please enter your EdUHK e-mail address in Question 24. Your EdUHK e-mail address will only be used for the Lucky Draw and for contacting you if you become a prize winner. It will not be linked to your answers and written comments.
2. Terms and Conditions for Lucky Draw:
   1. Only eligible respondents, i.e. EdUHK students and staff who have completed the questionnaire AND provided a valid EdUHK e-mail address are eligible to enter the Lucky Draw.
   2. EdUHK Library staff members are not eligible for participating in the Lucky Draw.
   3. Lucky Draw prize winners will be notified by e-mail using the EdUHK e-mail addresses supplied. Prizes must be collected in person from EdUHK Library.
   4. All Lucky Draw prize winners must be the holders of the EdUHK e-mail accounts provided for the Lucky Draw. They must also present their EdUHK Student Cards or Staff Cards when collecting their prizes.
   5. Each EdUHK student or staff member is entitled to enter once only in the Lucky Draw. Duplicate or multiple entries with the same EdUHK e-mail address will be disqualified.
   6. EdUHK Library reserves the right not to award prizes to anyone whose eligibility is in doubt.
   7. Decisions of EdUHK Library will be final.
   8. Lucky Draw results will be announced on EdUHK Library Website in late April 2023.
